Peterborough and Cambridgeshire`s Ambulance Service is asking relatives and carers of people living with dementia to fill in a form to help crews provide better care for sufferers.
It`s called This is me and provides an easy and practical way of recording who the person is.
Once completed the information will enable crews from the East of England Trust and other healthcare professionals who have contact with patients with dementia to see the person behind the condition rather than just the disease.
